RN Case Manager
StartDate: 2/23/2026 Pay Rate: $1800.00 - $2700.00
POSITION SUMMARY RN Case Manager
POSITION DUTIES Comprehensive clinical evaluation, assessment and documented care plan in all domains including social determinants of health, psycho-social, financial, medications, ADL/IADLs and cognitive, coordinating and monitoring all services needed with a focus on providing education and the goal of self-management skills.
MINIMUM REQUIRED QUALIFICATIONS GA license; Minimum five (5) years of RN Clinical Nursing, social work, case management or utilization management (UM).
SHIFT / HOURS PER WEEK M-F 8:00-5:00; Every other weekend; 40
